In 2022, HMDC experienced an unplanned separation of the Hibernia Southern Extension (HSE) water injection flowline weak link, resulting in an extended subsea water injection outage. As part of the response the team undertook a comprehensive Root Cause Failure Analysis (RCFA) to identify contributing factors and implemented a remediation strategy involving the removal of the failed weak link via ROV and installation of an ROV friendly connection solution. The presentation provides an overview of the key findings from the RCFA, the remediation approach, and highlights lessons learned used to enhance subsea reliability and reduce future risk across the fleet.
